Registration Forms

If you choose not to use the online registration system, you may download the appropriate registration form, fill out and fax or deliver to Arts Umbrella. Please note that registration done via the forms will be entered into our system up to 5-7 business days after they are received. 

General Registration form: for students aged 2-12

Teen Registration form: for students age 13-18
